Caution: Tough Training Road Ahead

Well folks, it looks like my training will be more difficult than I originally thought. After a series of tests, and numerous doctor visits, it has been determined that I have chronic sinusitis. I've been referred to an Ear, Nose, and Throat specialist who will look up my nose with a camera and tell me what options I have, but the doctor doesn't have openings until mid-November!
I've taken three rounds of antibiotics ranging from amoxicillin to sulfa drugs (which I found out I am allergic to) to levaquin, as well as a taper of prednisone. Unfortunately for me and my maxillary sinuses, nothing has helped. A CT scan revealed that I have moderate thickening of my right maxillary sinus with debris in the cavity and abnormal thickening of the ethmoid air cells. My septum also deviates severely to the left, contacting the right middle turbinate... Whatever that means!
Since the antibiotics and steroid treatments didn't work, it looks like surgery may be my only option. I'll get more information when I meet with my specialist in November, but I've already read a lot about chronic sinusitis and the treatment options. If I have the surgery, I will only be out of commission for a few weeks and then I should be good to go! But I can tell you the thought of surgery on my face really scares me! I read that they prefer to do the endoscopy under only local anaesthetic, but that general anaesthesia is an option, if you choose to pay for it. I think a couple hundred dollar to be asleep while they poke and prod at my sinus is a bargain! Who do I make the check out to?
